Before beginning the online enrollment process, please prepare all required documents to ensure a smooth and efficient registration experience at Shelomith Learning Center, Inc.
Basic Enrollment Requirements
Make sure you have the following documents ready:
PSA Birth Certificate
Report Card (Form SF9)
Certificate of Good Moral Character
Additional Requirements
Elementary and Secondary Levels
Applicants for Elementary or Secondary must also provide:
Completed Personal Data Sheet from SLC
Report Card showing no final grade below 80% in Science, Math, and English
General average of at least 80%
Conduct rating of B or higher
Letter of Recommendation or Certificate of Good Moral Character
Important Reminders
Ensure all required fields on the enrollment form are accurately completed to avoid processing delays.
Prepare clear scanned copies of documents for online submission. Original copies must also be submitted to the Registrar’s Office as required.





Online Enrollment
Enroll at Shelomith Learning Center from the comfort of your home with our simplified online process. Follow the steps below to ensure a smooth and successful registration for School Year 2025–2026.
Step 1: Complete the Enrollment Form
Download and fill out the official SLC Enrollment Form (SY 2526). You may complete it digitally or print and fill it out manually.
Step 2: Settle the Downpayment
Use our secure payment link or static QR code to pay via e-wallet or online banking. Take a screenshot of your proof of payment.
Step 3: Submit Required Documents
Submit the original copies of your Report Card (SF9), Certificate of Good Moral Character, and PSA Birth Certificate. You may hand-deliver the documents to the Registrar’s Office during the designated submission dates or email scanned copies to admin@shelomithlc.com for initial processing.
